From Idea to 50,000 Monthly Active Users: How Right Breathe Simplified Inhaler Prescribing in 6 Months

Inhaler Prescribing Was Broken

Clinicians across the UK faced a growing crisis.

Inhalers are among the most commonly prescribed medicines. But complexity in the market made it almost impossible for GPs to consistently choose the right one.

Patients were struggling too. Misuse, poor adherence, and mismatched prescriptions led to a cascade of health and economic consequences:

Emergency hospitalisations
Wasted medications
Overtaxed clinical teams
Billions lost in patient productivity

£150m

Estimated avoidable annual cost to the NHS from unused prescription medicines, with inhalers being a significant contributor

£4.5bn

Estimated cost to the UK economy from lost work, poor health, and wasted medication due to suboptimal inhaler prescribing

When Systems Break, Outcomes Suffer

Without the right tools:

  • Clinicians default to habit, not evidence
  • Patients misuse devices or give up altogether
  • The NHS pays twice: once for the medicine, and again for the preventable crisis

Everyone worked harder, but outcomes only worsened.

£125m

Annual cost of asthma emergency admissions to the NHS.

Thousands

Clinician hours lost on repeat appointments for inhaler issues

A Vision for Something Better

We knew the answer wasn’t just “better resources.”

It had to be a platform that worked at the point of care and in the patient’s hand.

Real-time, evidence-based prescribing guidance
Simple, visual technique training for patients
A scalable, secure platform that earns trust across the NHS

Enter: RightBreathe.

How We Built RightBreathe

Discover

We started with deep research to understand the landscape:

  • User and stakeholder interviews to uncover pain points, workflows, and unmet needs
  • Desk research and analysis of existing tools, NHS guidance, and clinical prescribing data
  • Problem mapping across both clinician and patient journeys
  • Behavioural insights to inform how support should be delivered at the point of care

This phase revealed where the current system was failing, and what a high-impact solution would actually require.

Define

With real-world insight secured, we defined the foundations for delivery:

  • Content and data standards: Defined the content attributes and scope for medicines and devices, aligned to the NHS Dictionary of Medicines and Devices (dm+d)
  • User stories and pathways: Developed comprehensive user stories for both clinicians and patients, mapped against real-world workflows
  • Agile delivery model: Established a transparent, flexible Agile structure that clinical, technical, and operational stakeholders were fully comfortable using

This phase ensured that every feature aligned with clinical reality and could scale with future needs.

RightBreathe sending me reminders to take my inhalers are the only reason why I still take them on time

Design

We brought the platform vision to life through intuitive, clinically-focused design:

  • Brand identity: Created a full visual identity, including logo, typography, colour systems, and brand guidelines
  • Vision and journey mapping: Developed key desktop and mobile screens aligned to critical steps in the user journey, uniting stakeholders around a clear product vision
  • Visual language and interface system: Designed an adaptable, clean UI capable of presenting complex inhaler information and clinical pathways to support quick, confident, and accurate decision-making

Every screen was crafted to reduce cognitive load and maximise action at the point of care.

I think it is a great tool for those new to respiratory / teaching

Deliver

We moved from vision to reality without losing clinical focus:

  • Custom CMS and UI development: Built a clean CMS and fully custom UI for managing medicines data, pathways, and resources
  • Integrations and pathways: Delivered real-time medicines selection integrations and embedded clinical pathways into clinician workflows
  • Cloud infrastructure and automation: Deployed the platform on secure, scalable cloud architecture with automated deployment pipelines
  • Real-world usability testing: Ran task-based user testing with clinicians across multiple respiratory care roles, refining the platform based on real-world feedback

The result: A scalable, secure, user-validated platform trusted by thousands of clinicians.

Fantastic app. Love the doses of the inhalers and if licensed for children and if it’s a low/medium/high ICS dose
